First start off by making a new canvas to work on. We will use a 3 x 3 inch in ours.
Now we are going to add some noise. Filter>noise>add noise.
Use the settings in Figure 1A. after that goto filter>blur>motion blur and use the
settings in Figure 1B.
(Your image should now look like Figure 1C)

Here we are going to bring out the detail in our barcode lines by adjusting the Images
levels. Goto image>adjust>levels and use similar settings to Figure 2A. Now we goto
filter>sharpen>unsharp mask and use the settings in Figure 2B.

Here we select the rectangular marquee tool from the toolbar (or press ‘m’)
And make a selection. choose a spot where your lines are best defined with sharp
Edges. Also, always start and end in a white line. Now we goto edit>cut (or ctrl+x).
(Figure 3A shows where I thought was best to make my selection.)
Once you have cut out your barcode goto file>new (or crtl+n) and paste your barcode
into your new canvas. Edit>paste (or ctrl+v). that’s pretty much it. You can add some
Details like I did in Figure 3B by adding a new layer and filling a small rectangle filled
with white near the bottom. Now you can type numbers or whatever else you want in
the text area.
